What's New in Aging at Homeundefined 

Year 3 – At its February 2010 meeting, the HNHB LHIN board approved six proposals for Year 3 of the Aging at Home (AAH) Strategy, which is designed to find innovative ways to help seniors age independently at home and avoid unnecessary visits to the emergency department. At this time, the proposals are before the Ministry of Health and Long-Term Care for final review.

Year 2 - In September 2008, all LHINs received direction from the Ministry of Health and Long-Term Care that Year 2 Aging at Home funds were required to be linked to improved patient flow, with a focus on a)  alleviating ALC levels and, b) emergency department diversion.  We are developing a comprehensive ALC alleviation and ED diversion strategy to meet our ALC target of 16%.   To date, 10 initiatives have been approved for funding for Year 2 (2009-10).  The remainder of the funding for this year will be announced at a later date.  Funding for approved projects will flow shortly.  You can read the media release and backgrounder for more information. 

Year 1 - Funding for 32 Year 1 (2008-09) Aging at Home initiatives flowed in August 2008, and many were funded on an one time recurring basis, to allow for some evaluation prior to committing funds to base.   Evaluation occurs on a quarterly basis by tracking the initiatives performance measurement targets.  Decisions about reallocation are ongoing.

  • September 5, 2008:  In May 2008, the HNHB LHIN surveyed providers of supportive housing across the LHIN to find out how many people live in supportive housing at present, and how many more need supportive housing.  This information was used to create a Brief on Supportive Housing for Hamilton, Niagara, Haldimand, Norfolk, Burlington, and Brant  which has been posted under Supportive Housing/Supports in the Home.
