Aiming for Better Health

The HNHB LHIN has adopted a planning framework for system level planning - Triple Aim, and will be piloting this framework within Chronic Disease Prevention and Management (CDPM). CDPM is an HNHB LHIN priority as outlined in our Integrated Health Service Plan (IHSP) 2010-2013.

What is Triple Aim?Triple Aim Triangle

Developed by the Institute for Healthcare Improvement (IHI), Triple Aim includes three components:

  • Improving the health of the defined population,
  • Enhancing the patient care experience (including quality, access and reliability), and,
  • Reducing or at least controlling the per capita cost of care.

The three components of Triple Aim are interconnected - a change in one component can affect the other two.

Why?

The Triple Aim approach aligns with the HNHB LHIN's vision and IHSP priorities.

This approach includes roles for both macro and micro integrators. Macro integrators are those that can pull together resources of numerous organizations to form a virtual system to support and ensure the system is optimized for a defined population. The micro integrator ensures that the best and most appropriate care is provided to all. The Triple Aim approach has a proven track record for success.

How will we apply it?

The LHIN will apply Triple Aim to its priority area of Chronic Disease Prevention and Management (CDPM). Based on extensive quantitative and qualitative analysis and together with the LHIN's CDPM Collaborative Advisory Committee, three communities were chosen (Fort Erie, Norfolk and downtown Hamilton) to pilot Triple Aim to address CDPM issues. Health status indicators, such as prevalence of diabetes and chronic obstructive pulmonary disease (COPD), and health care usage, demonstrated an opportunity for these communities to reap the potential benefits from applying Triple Aim.

A pilot of Triple Aim will begin in Fort Erie, followed by a phased approach in Norfolk and downtown Hamilton, where citizens and stakeholders will have the opportunity to participate in the development and planning of local CDPM priorities and action planning. Work is underway in preparation for the Fort Erie pilot.
