In the winter of 2011, the HNHB LHIN assembled a Senior Friendly Hospital Strategy Task Group to help guide work on the LHIN's priority of improving seniors’ health through a model of Senior Friendly Hospital care. By June 2011, all 14 LHINs will have conducted a similar process so that the provincial landscape of Senior Friendly Hospital care may be surveyed and the development of a province-wide Senior Friendly Hospital strategy can commence.
Jun 30, 2010 An Integrated Program for Complex Care in the HNHB LHIN
At its June 29, 2010 meeting the HNHB LHIN Board of Directors approved a plan to create an LHIN-wide integrated program for complex care. The plan includes seven recommendations.

Following a review of Year 1 proposals submitted for funding for the Aging at Home (AAH) Strategy, the HNHB LHIN Board recognized the need to have Specialized Geriatric Services priorities that could be used to assist them in making future funding decisions related to Year 2 and 3 AAH proposals. The Geriatric Access and Integration Network (GAIN) was asked to lead this process.

The HNHB LHIN held a series of open houses across the LHIN in spring 2006. The sessions provided an opportunity for LHIN Board members and senior staff to meet people and hear what they had to say about their health system, and to introduce the role of the LHIN. Close to 700 people attended the open houses.

Health planning and decision-making in Ontario will be carried out 'close to home', by those who know and understand their area health system best; the Ministry of Health and Long-Term Care is devolving centralized authority and accountability for the local health system.  The advent of LHINs signals shared responsibility for health system improvement, and a shift from a culture of expectation of funders, providers or policy makers alone to 'fix' health care.  No one stakeholder carries sole responsibility for a sustainable health care system.  Accountability for a responsive and sustainable health system will be shared among citizens, stakeholders, providers and government.
Feb 01, 2005 System Integration Opportunities for LHIN 4, February 2005
Prepared by the Project Steering Committee for Local Health Integration Network 4, this report summarizes ten initiatives for improving the coordination and integration of health services within the LHIN.  The ten initiatives were  selected from among almost 50 initiatives proposed during the course of a full-day workshop held in November 2004.  The workshop was attended by 300+ health care stakeholders from across the LHIN area.  The Hamilton Niagara Haldimand Brant LHIN adopted these ten initiatives as their early integration priorities.  As such, these initiatives have provided the foundation for planning that is ongoing.
